Baricitinib Shows Sustained Response in 330-Patient Alopecia Areata Cohort

Key Clinical Summary

·  Baricitinib demonstrated substantial real-world effectiveness: Among 330 patients with alopecia areata, alopecia totalis, or alopecia universalis, 61.4% achieved SALT ≤20 and 27.4% achieved complete hair regrowth (SALT 0). Trichoscopy detected vellus hair regrowth as early as 4 to 5 weeks, with significant regrowth generally occurring within 2 to 6 months.

·  Pediatric patients experienced comparable outcomes, with greater responses observed with younger age and shorter disease duration: Among 135 pediatric patients, 61.7% achieved SALT ≤20 and 32.3% achieved SALT 0. Complete regrowth occurred in 40.6% of children aged 2 to 6 years, while 74% of patients with disease duration under 6 months achieved SALT ≤20.

·  Long-term tolerability was favorable: Adverse events were uncommon and primarily mild, including upper respiratory tract infections (1%), acne (0.6%), hyperlipidemia (0.3%), and transient liver enzyme elevation (0.3%). No treatment-limiting tolerability signals, deep vein thrombosis, or pulmonary embolism were reported, although the retrospective, single-center design and absence of a control group limit interpretation.

Long-term real-world data suggest baricitinib provides substantial hair regrowth with favorable tolerability in patients with alopecia areata (AA), including children and patients with alopecia totalis or alopecia universalis.

Study Findings

The investigators conducted a retrospective, single-center study of 330 consecutive patients with AA treated with baricitinib at West China Hospital between August 2020 and June 2024, with follow up through June 2025. Patients ranged from age 2 to 63 years, and 40.9% were pediatric patients.

Patients received baricitinib 2 mg or 4 mg once daily based on clinician discretion and treatment response. The primary endpoint was achievement of a Severity of Alopecia Tool (SALT) score of 20 or lower, representing at least 80% scalp hair coverage.

Overall, 61.4% of patients achieved SALT 20 or lower, whereas 27.4% achieved SALT 0, indicating complete hair regrowth. Trichoscopic monitoring identified vellus hair regrowth as early as 4 to 5 weeks after treatment initiation, with substantial regrowth typically occurring within 2 to 6 months.

Clinical Implications

Pediatric patients had comparable outcomes, with 61.7% achieving SALT 20 or lower and 32.3% achieving complete regrowth. Among children aged 2 to 6 years, 40.6% achieved SALT 0. Patients with disease duration shorter than 6 months also demonstrated favorable outcomes, with 74% achieving SALT 20 or lower, “highlighting the importance of early intervention in AA management.”

Adverse events were mostly mild and infrequent, including upper respiratory tract infections, acne, hyperlipidemia, and transient liver enzyme elevations. No deep vein thrombosis, pulmonary embolism, or treatment-limiting tolerability signals occurred.

Expert Commentary

The investigators concluded that the findings provide “additional evidence to support the use of baricitinib in routine clinical practice,” while emphasizing the need for controlled pediatric studies and dose optimization.
