The randomized P4 trial, presented by Carlos Collet at TCT 2026, compared coronary CT angiography (CCTA)-guided and intravascular ultrasound (IVUS)-guided PCI strategies in 1,105 patients.
- At 12 months, target vessel failure occurred in 4.1% of the CCTA group and 4.6% of the IVUS group, meeting the trial’s primary endpoint of noninferiority.
- CCTA guidance was also associated with shorter median procedure time (46 vs 63 minutes) and lower median contrast volume during PCI (150 vs 170 mL).
