Emerging Diagnostics and Targeted Therapies Reshape Interstitial Lung Disease Management

Recent advances in imaging, molecular diagnostics, and targeted therapies are transforming the clinical landscape of interstitial lung disease (ILD), particularly idiopathic pulmonary fibrosis (IPF), according to a comprehensive review examining evolving diagnostic and treatment strategies. The authors highlight how new technologies may improve disease characterization, prognostication, and therapeutic decision-making.

High-resolution computed tomography (HRCT) remains the diagnostic cornerstone for ILD. However, the review emphasizes growing interest in complementary approaches, including genome analysis and epigenetics in IPF, circulating biomarkers, robotic bronchoscope navigation, and transbronchial lung cryobiopsy. These methods aim to refine diagnostic accuracy and reduce uncertainty in complex cases. The authors note that the ILD field is “rapidly expanding as new insights highlight novel mechanisms and procedures that influence epidemiology, diagnosis, and treatment.”

The review also addresses IPF-associated pulmonary hypertension (Group 3) and the development of clinical scoring systems to better define disease progression. Advanced imaging modalities such as positron emission tomography/computed tomography are discussed as potential tools for disease assessment, while prostacyclin therapies and antifibrotic agents are evaluated for their roles in management. Lung transplantation remains a key option for selected patients with end-stage IPF.

Beyond established disease, the authors examine interstitial lung abnormalities (ILA) and the intersection of IPF with lung cancer. They also describe the emerging role of artificial intelligence in ILD diagnosis and longitudinal monitoring, noting its potential to enhance image interpretation and risk stratification.

The authors conclude that integrating novel diagnostics with established imaging “may improve diagnostic accuracy” and support more individualized management strategies. They also underscore the importance of understanding disease progression and associated comorbidities to guide therapy selection.
