AI-Driven Documentation Enhances Diagnostic Accuracy and Workflow Efficiency
Regard’s upgraded artificial intelligence (AI) platform is helping doctors diagnose patients more accurately at the bedside by combining real-time conversations with comprehensive patient chart data, according to an article from Fierce Healthcare.
The new “proactive documentation” feature integrates data from electronic health records with ambient dictation during patient visits to generate near-complete draft notes before and during encounters. This dual approach captures both historical and up-to-the-minute clinical information, enabling physicians to identify missed conditions such as hypertension, malnutrition, and sepsis. “It’s the first product to intelligently merge conversation insights with medical record data into a complete note,” said Eli Ben-Joseph, founder and CEO.
Early testing at health systems like WakeMed has shown the tool to be a game-changer in surfacing overlooked details without adding to physicians’ workload. In settings where patients are often managed by multiple teams, like in the intensive care unit (ICU), important information can be lost. WakeMed’s chief clinical integration officer, David Kirk, MD, noted that the technology not only helps capture missed diagnoses but also supports ethical practice by acting as a “double-check” for clinicians. The hospital system has expanded use of the tool from a single pilot site to all three hospitals, with plans to make it available systemwide over the next year.
Beyond diagnostics, the platform supports quality measures and accurate coding for reimbursement, which is critical for inpatient and ICU care. By producing more precise documentation without requiring extra time in the chart, the system helps hospitals reflect the true complexity of patient cases, improving quality scores and revenue. Dr Kirk emphasized the win-win nature of the platform, stating, “The patient gets better care, the doctors are happy, and hospitals benefit from improved quality scores and revenue.”
