New Visual Scale Aims to Standardize Nail Psoriasis Severity Assessment

A newly developed visual tool may improve consistency in assessing fingernail psoriasis severity, addressing a longstanding gap in clinical trials and practice, according to a qualitative validation study introducing the Visual Medical Scale to Evaluate Nail Psoriasis Severity (ViSENPsO).

Nail involvement is common in psoriasis and is associated with functional impairment and reduced quality of life, yet standardized severity measures remain limited. ViSENPsO was designed as an illustrated 5-point scale to provide a practical and reproducible method for grading disease severity.

The development process involved a global panel of dermatologists who participated in concept elicitation interviews and image-based ranking exercises. These steps were used to define clinically meaningful severity categories based on histopathology-confirmed nail images. Follow-up cognitive interviews assessed clarity, usability, and content validity of the scale.

The study demonstrated strong agreement among participants in categorizing nail severity, supported by high competence scores during ranking exercises. Preliminary interrater reliability testing showed moderate-to-strong agreement, with an intraclass correlation of 0.74.

The authors reported that “ViSENPsO is a validated, visually guided tool for assessing standardized nail psoriasis severity,” and noted that “preliminary interrater agreement was observed,” supporting its potential utility in clinical and research settings.

Although full psychometric validation is ongoing, including assessment of sensitivity to change, early findings suggest the tool may improve consistency across evaluators.

As validation progresses, ViSENPsO could become a useful adjunct in both research and clinical practice, helping to address variability in nail psoriasis assessment and enabling more precise evaluation of treatment response.
