Negative Expectations Linked to Brain Structure, Depression Course in MDD

Negative expectations about the future were associated with differences in brain structure and a longer course of depressive illness among patients with MDD, according to findings published in the Journal of Affective Disorders. Greater gray matter volume in the rMFG was observed among patients with negative expectations and predicted the duration of subclinical depressive episodes over 2 years.

Study Findings

Negative expectations are often observed among patients with MDD and have previously been linked to less favorable outcomes, including poorer treatment response and suicidal behaviors. However, the neurobiological characteristics associated with negative expectations remain unclear.

Researchers compared structural MRI measures and 2-year longitudinal clinical data among 330 patients aged 18-65 years with a DSM-IV-TR diagnosis of MDD. Patients were divided into groups based on the presence or absence of negative expectations, assessed using item 2, “pessimism,” of the Beck Depression Inventory-I (BDI-I).

The groups were matched for age, sex, depressive symptom severity as measured by the Hamilton Depression Rating Scale (HAM-D), and global functioning as measured by the Global Assessment of Functioning scale. 

Structural MRI analysis showed that patients reporting negative expectations (n = 165, BDI-I item 2 scoring 1–3) had increased gray matter volume in the rMFG (k = 177, t = 3.97, x/y/z = 42/16/36, p = .043, FWE, partial ŋ2 = 0.022) compared with patients without negative expectations (n = 369, BDI-I item 2 scoring 0).

Differences also emerged in the subsequent course of depression. Patients who expressed negative expectations at baseline experienced significantly longer depressive episodes during the 2-year follow-up period (p < .001, d = 0.667, corrected for multiple comparisons).

Additionally, rMFG volume significantly predicted the average duration of subclinical depressive episodes during follow-up (F(7, 173) = 1.02, p = .042, R2 = 0.04, (adj. R2 = 0.039)). 

Clinical Implications

The findings suggest that negative expectations in MDD are associated not only with subsequent illness course, but also with measurable differences in brain structure. The patient groups were matched for baseline depressive symptom severity and global functioning, allowing investigators to examine differences associated specifically with the presence or absence of negative expectations.

The longitudinal findings further showed that baseline negative expectations were associated with longer depressive episodes over the following 2 years, while rMFG volume predicted the duration of subclinical depressive episodes.

The authors indicated that these findings could provide a foundation for future research examining therapeutic interventions that target patient expectations as a potential way to influence MDD outcomes.

“These results support stratifying depressed patients regarding clinical or biological features to better capture variation in the course of illness and design psychotherapeutic interventions to specifically target subgroups,” wrote Laila Noor, MD, Department of Psychiatry and Psychotherapy, University of Marburg, Marburg, Germany, and study coauthors.

The authors concluded that the findings suggest “an intriguing avenue for future studies, encouraging the exploration of neurobiological correlates of depressive symptoms in subgroups regarding their cognitive vulnerability and risk factors.”
