(#193) The Impact of Pharmacists in Multidisciplinary Home Medication Review Services for Psychiatric Patients at Hospital Kuala Lumpur

Abstract: Introduction

Drug adherence and drug related problems (DRP) are common challenges in the treatment of serious mental illnesses (SMI). Home medication review (HMR) by pharmacists plays an important role in ensuring appropriate medications are used and enhancing adherence among patients with SMI.

Objectives:

This study aimed to evaluate the impact of HMR pharmacists in identifying DRP, potential side effects and adverse effects encountered by psychiatric patients as well as the impact in medication adherence among psychiatric patients.

Methods:

A retrospective cohort observational study was conducted to gather HMR reports from psychiatry HMR services in Hospital Kuala Lumpur from 1st September 2023 to 31st August 2024. All HMR reports were conveniently sampled into this study.

Results:

A total of 364 patients were recruited into this study. The most frequent DRP identified were patient-related causes, in which 47.3% of them intentionally consumed medications less than prescribed and 13.7% had no or inappropriate outcome monitoring. 21.3% and 16.3% of them reported hypersalivation and oversedation, respectively, as the main adverse drug events. The acceptance rate of pharmacist's recommendations made during HMR visits was 73.3%. Among different prescribed treatment regimens, those prescribed with a combination of oral and long-acting injectable medications showed the poorest adherence rate (50.4%), followed by oral medications only (33.3%) and long-acting injectables only (10.6%).

Conclusions:

Pharmacists should actively review patient's medication regimen during each HMR visit to recognize DRP encountered by patients and improve their treatment adherence.

Short Description: This is a study investigating the impact of pharmacists in multidisciplinary home medication review (HMR) services for patients with serious mental illnesses (SMI). This study identified common drug-related problems and adverse effects encountered by patients taking psychotropic medications. In addition, adherence rates among patients with SMI receiving HMR services were identified, specifically scrutinizing into patients receiving different treatment regimens such as oral-only, long-acting injectables-only and a combination of oral and long-acting injectables medications.
