Introduction/Background/Significance: Over a decade, significant changes in the management of patients with Chronic Lymphocytic Leukemia (CLL) had occurred with respect to diagnostic and therapeutic perspectives. Diagnostic approaches moved from flow cytometry toward molecular cytogenetic and immunoglobulin heavy chain mutation analysis that defined the disease and treatment options. At the same time, treatment choices moved from chemotherapy (CT) and chemo-immunotherapy (CIT) based therapies to B cell receptor (BCR) molecular targeted therapy with the main use of Bruton Tyrosine Kinase Inhibitors (BTKi); namely Ibrutinib & Acalabrutinib & BCL2 inhibitor namely; Venetoclax

Materials and Methods/Case Presentation/Objective: Retrospective analysis of molecular cytogenetic characteristics of patients with CLL diagnosed over a decade. IRB Approval number is IRB-2023-112 and there is no conflict of interest.

Results/Description/Main Outcome Measure(s): Over a decade, (2014-2024), 41 patients were identified, 16 women and 25 men, with a median age off 60-70. Of these 41 patients 29/41 (71%) of CLL patients had FISH analysis done and only 3/29 (10%) had 17p del/TP53 mutation. Majority of patients had 13q- deletion 17/29 (59%). Of these 41 patients 13 (32%) of patients had IGHV mutation analysis done.  Of these patients who had IGHV mutation analysis done 9/13 (70%) were unmutated IGHV, while 4/13 (30%) had mutated IGHV. This means that two thirds of tested CLL patients had a worse molecular prognostic marker which will affect the treatment plan decision of avoiding CT or CIT and these patients should be treated with BTKi of Ibrutinib or Acalabrutinib +/- BCL2i.

Conclusion(s).: Molecular Cytogenetic Characteristics of CLL patients impacted treatment choices
