Patient Case:

A 57-year-old non-smoking patient with metastatic lung adenocarcinoma harboring a HER2 exon 20 mutation presented for a second opinion after progressing on frontline carboplatin, pemetrexed, and pembrolizumab after 3 months of therapy.

Restaging imaging demonstrated disease progression with enlarging bilateral pulmonary nodules, new hepatic and osseous metastases, and more than 25 new brain metastases, which were subsequently treated with whole-brain radiation therapy.

Repeat next-generation sequencing (NGS) confirmed the presence of an ERBB2 Y772_A775dup (exon 20 insertion) mutation, along with an NF1 alteration.
