Targeting KRAS G12D in Metastatic Pancreatic Cancer After First-Line Progression

Patient Case:
A 62-year-old man presented with metastatic adenocarcinoma of the pancreatic body and tail, with biopsy-confirmed liver metastases. He started first-line gemcitabine/nab-paclitaxel. After an initial period of stability, restaging showed hepatic progression, with a dominant liver lesion growing from 1.2 cm to 4.2 cm, accompanied by a rise in CA 19-9 from 103 to 8,530. ECOG is 1. Liver enzymes are within normal limits.
Somatic NGS testing identified a KRAS G12D mutation with a co-occurring TP53 mutation.
